Intractable Bloody Pleural Effusion in A Rheumatoid Patient

Abstract

A 34 year old man was admitted for the left pleural effusion and dyspnea on exertion. He had been medicated for rheumatoid arthritis, diagnosed 4 years ago, but he stopped it for 3 months by himself recently. The pleural effusion was drained by a thoracentesis and bloody slightly. We could not find the causes despite of chest CT imaging and testing the drained pleural fluid, considering an infectious condition, malignancy and other connective tissue diseases. Also we performed VATS to exam the thoracic cavity and pleura microscopically. The pleura was revealed only a chronic inflammatory lesion nonspecifically. We suspect it as a complication of rheumatoid arthritis and medicated him steroid and methotrexate. The pleural effusion was decreased and followed up for 5 months without recurrence. As this patient, the bloody pleural effusion as a complication of rheumatoid arthritis is rare. Therefore we report this case with literature review.
